Just a thought, but there are some companies that for about $20 make programmable gamepads that go inline with the keyboard connector.
I had one (that finally died after three years) with essentially an expanded PSX controller layout -- 12 programmable keys + programmable D-pad buttons, all mappable to the keyboard or to keyboard sequences! All that was necessary to program it was to hold a "program" button on the pad, hold down the desired key, and then tap a key or short key sequence on the keyboard. Then release the "program" button and...bingo.
Maybe if you could get your hands on one, you could disassemble it from the plastic case and rebuild it to your liking...I think I bought mine at a KMart store but I haven't looked for one in a while.
