SQL server 2005 express edition is used by many software packages. It usually gets installed with such software. I assume it is the "express edition" because you say the SP3 update is 50MB. For the full version the update is 300MB to 1GB.

If your machine is protected by one or more firewalls at all times you are probably fine not upgrading, otherwise it would be a good idea to do the upgrade. MS SQL has had several secuirty issues ofer the years.

Another option is to find out which application installed/needs SQL and, if your not using said application, uninstall it.
