If you have all the tools on hand and basic building and soldering experience, it shouldn't take more than a few hours to construct on perfboard. Cost will depend on whether you have any of the hardware on hand, and where you shop for any stuff you don't have.

The TL072 is a dual JFET-input opamp that is more than adequate for most general-purpose audio preamping applications (hifi, too, provided you're not one of those Class A purists or some such). In your case, you will need a total of eight amp channels, so you could use four TL072s or two TL074s.

The 10uF capacitors should preferably be bipolar electrolytics, or substituted with 1uF polyesters. The opamps are spec'd for a power supply as high as +/-36V, so you have some flexibility on the transformer -- although if you go much higher on the supply voltage you may want to change the 10k log pots over to 100k log.
