SupraExpress 56 supports ShotGun in addition to the SupraSonic which is a 2 in 1 ISA board. Express should be available in PCI, and it's supposed to work with other modems as long as one of them is a SupraExpress.

I don't know about the MultiLink in Windows though. Also, some ISP's charge extra (but try it first, let them figure it out if they can).
