CD-RW won't record

Hi, I have a Yamaha CD-RW and windows 98. It is about 2 years old and I've recorded approx 12 disks with it. Now when I try to record using Adaptec Easy CD Creator Copier Deluxe it either says "no media inserted" or starts then pops up with "no disk in CD recorder". Sometimes the door won't open then and I have to restart to get disk out. Sometimes it freezes up the whole computer and I have to reset. Sometimes it tests 10 tracks (music cd) then pops up with T0100 I/O error (04/09/00) -CD recorder error- track following error - ("help" says the reason is the recorder is having trouble following the track) and it also says D0100: I/O error (06/28/00) Status disk may have changed. "help" doesn't have any idea what that means. I've cleaned it thoroughly I even took it apart and cleaned the silver "head"? thing with an alcohol Q-tip like you do VCR's. As you can see I'm getting desperate. It does seem to try harder to start after I clean it over and over. But still ends up with errors and won't record. Anyone have any idea what can be wrong with it? Any suggestions would be greatly appreciated. Thanks in advance! P.S. It still will play music CD's with "CD Player"
