With a Diamond Viper V770 Ultra. I had it replaced, and the new card did the exact same thing. The only way I could find to fix the problem was to underclock the memory speed of the card down to about 175 (default is 183). It seems to me that the "Ultra" is just an overclocked version of the TNT2, and apparently the overclock is too extreme for some of the boards.
